Room Analysis Report

Each room that MAS analyzes gives you irrefutable documentation as in the report  seen below.  We can perform our "Basic", "Standard" or "Standard PLUS" analysis.  Each analysis will enable us to certify a room for JCAHO and OSHA as long as all requirements are met.  For "due diligence" purposes, we strongly recommend that you select the "Standard" analysis and certification as your preference.


Standard Report Parameters

The report is issued to the individual in the facility that has requested and authorized the funds to conduct the analysis or certification.  MAS will provide two (2) copies of this report.  If more copies are required it should be requested on the purchase order or requested at a later date by contacting the corporate office.

Some reports will have more or less fields depending on the design and layout of the area being surveyed.  This sample is representative of a standard isolation room.  Surgical suite report can be very different as can other negative or positive pressure rooms survey result forms.

Room Description

Room designation, usage and any other information needed to identify the room.

Room Pressure Positive or Negative.  N/A= Ambient pressure room.
Press. Differential (inches of water)

A negative or positive pressure reading taken in relation to the area outside the primary entrance/ egress to the room (in "W. C.).

Grandfathering A notation as to whether the room falls under JCAHO's grandfathering provision for infectious isolation rooms.
Air Velocity Under Room Door (fpm)

The amount of offset (make-up) air measured in feet per minute.  When calculated in to cubic feet per minute (CFM) it should be no less than 100 CFM.

Measured Air Changes / Hour (ACH)

The calculated ACH based on airflow supply and exhaust.  Subset data includes room dimensions and air flow measurements.

Ventilation Efficiency

For negative pressure rooms only. Particle decay analysis at each side of the patient bed, table or test area.  This number will exceed 100% since only the area where the patient resides is tested.  Result should be >100%.

Adjusted ACH

This number represents the actual ACH in the immediate vicinity of the patient.  For negative pressure rooms only.

Air Changes per Hour (ACH) - Bathroom (if applicable)

The calculated ACH based on airflow supply and/or  exhaust.  Subset data includes room dimensions and air flow measurements.

Bathroom Pressure

Positive or Negative in relation to the area outside the entrance/egress door.  N/A= Ambient pressure room.

HVAC Air Supply - Prefiltration

Filter rating for prefilter at the main air handling unit for negative pressure rooms or final filter before air enters a positive pressure room.

HVAC Air Supply - Main (final) Filter

Filter rating of bag, box, HEPA or other filter(s) at the main air handling unit for negative pressure rooms or final filter before air enters a positive pressure room.

Room Air Exhaust - Prefiltration

Filter rating for prefilter at the exhaust unit for negative pressure rooms or final filter before air exits a positive pressure room.

Room Air Exhaust - Main (final) Filter

Filter rating of bag, box, HEPA or other filter(s) at the main air handling unit for negative pressure rooms or final filter before air exits a positive pressure room.

Distance of outdoor exhaust to pedestrian area or building orifices.

If exhaust from a negative pressure room is unfiltered, a minimum of 25' is required between exhaust and any pedestrian areas or openings (doors, air intakes, windows, etc. - For negative pressure rooms only).

Warning Labels on Exhaust Ducting

Warning labels are required on all ducting that exhaust air from negative pressure isolation rooms.

Outdoor Air Changes per Hour

The Engineering Department should confirm a minimum of 2 ACH of fresh air for critical care areas.

Noise Level at Patient Head Level

Noise level is measured at the patient's head area to ascertain the noise comfort level.  This is important if there are modular air purification units being utilized in the room.  Measured in dB.

Noise Level at Three (3) ft.

Performed on any air supply and exhaust equipment including modular air purification units.  Measured in dBA.

Radiation Level

Measured in millirads.  This test includes areas where nuclear materials are or have been in use to detect any residual radioactive levels.

Supply HVAC Air to Room

Total volume of air supplied to the room including the type of diffuser utilized.

Exhaust HVAC Air from Room

Total volume of air exhausted from the room including the type of exhaust method and/or equipment utilized.

Difference - Exhaust  vs. Supply (cfm)

The difference between the supply and exhaust in the room.  The minimum should be 100 CFM.

Percent Higher than Exhaust Air

The difference of exhaust versus supply represented in a percent value.

Bathroom Supply Air

Total volume of air supplied to the bathroom including the type of diffuser utilized. (Most bathrooms do not have a supply diffuser)

Bathroom Exhaust Air

Total volume of air exhausted from the bathroom including the type of exhaust method and/or equipment utilized.

Carbon Dioxide Level Total CO2 level expressed in parts per million (ppm).
Carbon Monoxide Level Total CO level expressed in parts per million (ppm).
Temperature Room temperature in degrees Fahrenheit.
Relative Humidity Relative Humidity (rH).
dP Monitoring Method

Must be a local, continuous monitor with both audible and visual alarming capabilities.

Monitoring Records Available?

A dP reading must be taken and recorded for all occupied infectious isolation rooms each day.  An infectious isolation room not being utilized for infectious isolation must be taken and recorded once per month.

Page 5 - Particle Decay Analysis Data

This page gives a graphical representation of the data collected to determine the ventilation efficiency.  It takes the actual versus calculated air exchanges to give efficiency expressed as a percentage based on the "ideal" efficiency determined by the CDC.

This analysis is only done in the patient retirement area.  This is the area most important for the safety of the health care worker, service personnel or visitors that may enter the room.

High Priority Items

The colored blocks on the Measurement and Assessments pages will be coded red for any problem that would preclude certification.  Suggested remediation will be provided in this space.

 
Medium Priority Items

The colored blocks on the Measurement and Assessments pages will be coded yellow for any problem that would compromise safety for the patient and/or the health care workers.  Suggested remediation will be provided in this space.


The Basic Analysis, which is a three (3) page report, will determine just the air flows in and out of the room with calculated ACH along with pressure readings and a few other basic parameters.

The StandardPLUS analysis will give you all that the Standard Analysis gives you plus air gas analysis in the room complete with a report demoting any remediation measures needed to correct any problems that are discovered.  You can also opt to have a pathogen analysis to check for any airborne or surface microbes that may be present.

This irrefutable documentation is sent to you in duplicate so that both the engineering department and infection control can maintain copies for their files.  There is also a certificate suitable for framing that is provided for each room.
